Outline of Research at the Start

日本および南・東南アジア（主にバングラデシュ、ミャンマー）を対象とし、最近新しい菌種として報告されたブドウ球菌種Staphylococcus argenteus、市中感染から分離される市中感染型MRSA（メチシリン耐性黄色ブドウ球菌）、抗MRSA薬に耐性を示すブドウ球菌について、それらの分布の実態、薬剤耐性度、薬剤耐性のメカニズムについて解析する。得られた情報に基づき、薬剤耐性ブドウ球菌の拡がりを抑える方策を検討する。

Outline of Annual Research Achievements

日本（北海道）における血流感染由来MRSAは2019年8月から2年間で279株を収集・保管し、解析を行った。遺伝子型は、clonal complex (CC) 5（ST5/ST764）とCC1が各々47％、42％を占め、過去の結果に比してCC1-SCCmecIVaの増加が顕著であった。7株のST8-SCCmecIVa株はPVL遺伝子陽性（2.5％）で、うちACME陽性の6株は米国で優勢なUSA300クローンであると考えられた。ACME陰性の1株の全ゲノム配列を解析したところ、これはUSA300クローンからACMEが脱落した変異株であることが示唆された。さらに2023年4月から12月まで収集した血流感染由来MRSAの解析ではCC1が60％を占め、市中感染MRSAにおいてCC1の増加傾向が顕著に認められた。今年のアジアでの疫学調査はバングラデシュにおいて行った。共同研究機関のマイメンシン医科大学で2年間で分離された170株の黄色ブドウ球菌を解析したところ、MRSAは36％でありその8割がSCCmecIVを保有していた。PVL遺伝子陽性率の高さが特徴的であり、MSSA（62％）のほうがMRSA（26％）より高かった。MRSAの中にUSA300クローンが1株検出され、リネゾリド耐性を付与するcfr遺伝子がMSSA1株で検出された。またS. argenteusが2株同定（ST2250）され、これはバングラデシュで最初の報告となった。北海道におけるS. argenteusの収集、解析は2019年以降、2023年末まで継続しているが、ST1223、ST2198、ST2250の3遺伝子型のみ同定され、年次の経過とともにST2250の相対的な増加傾向が認められた。
