Outline of Research at the Start

シグマ-1受容体が左室心筋肥大において治療のターゲットになりうることが報告されている。そこで心筋梗塞後の修復過程においてこの受容体の心筋での発現がいかに変化してゆくかを画像化するための放射性トレーサーを開発する。救急医療での経皮的冠動脈形成術(PCI)による再灌流療法を模したラット心筋虚血再灌流モデルにおいて、画像化をオートラジオグラフィおよび生体イメージングであるSPECT/CTで行い、梗塞後の継時的な変化を評価する。治療的介入による受容体の発現の変化、受容体刺激薬がもたらす心筋保護作用の有無、また受容体発現程度と治療効果の関連についても検討し、イメージングの意義を考察する。

Outline of Final Research Achievements

To image the spatiotemporal expression of sigma-1 receptor (Sig-1R), we made the ligand, OI5V, that have high affinity for Sig-1R. Three to 7 day after 30min myocardial ischemia and reperfusion, I-125-OI5V accumulation corresponded to the area with reduced myocardial viability demonstrated by reduced Tl-201 uptake in area at risk and with macrophage infiltrations or adjacent to macrophage infiltrations area. Therefor, OI5V imaging is considered useful method to evaluate damaged myocardium by ischemia. In rats with doxorubicin treatment, OI5V uptake of the heart declined before 2 weeks earlier than the beginning of the left ventricular ejection fraction deterioration. Then the OI5V imaging should be useful for the monitoring the doxorubicin induced cardiomyopathy.

Academic Significance and Societal Importance of the Research Achievements

Sig-1Rは元々脳で発見された受容体である。近年心臓にもその発現が報告され、種々の病態変化に関与し、治療のターゲットとなる可能性が示唆されているが、梗塞心筋や抗がん剤による心筋障害での画像化は未だ未知の領域であった。そこでSig-1Rに高い親和性を示す、OI5Vを開発し、ラットの梗塞心筋、doxorubicin心筋障害モデルを用いてそのイメージングの意義を示すことができた。心筋梗塞では心筋viabilityが低下し、macrophage浸潤の関与する部に集積し、doxorubicin心筋障害では心機能障害の発生2週間前より心筋集積が低下することが画像として示された。
