Outline of Research at the Start

肺全体では移植に不適応と判断されたドナー肺でも,肺葉によっては良好な状態を維持し,移植可能であることがある.しかしながら,肺移植におけるグラフト機能評価では局所の呼吸機能を測定することは難しく,いまだに確立された肺葉ごとの評価方法はない.本研究の目的は,ドナーにおけるin vivoと体外肺灌流装置におけるex vivoでの局所呼気CO2濃度測定が,ドナー肺葉ごとの移植適応評価として有用であるかを検討することである.

Outline of Final Research Achievements

We established a lung lobe grafting model using Yorkshire domestic pigs, introduced an aspiration pneumonia donor model, and established a method for evaluating lung lobe grafts. In the first experiment, we established a model of lung lobe grafting in smaller pigs using lung lobes removed from larger pigs. Next, after injecting gastric juice into the airway of donor pigs and introducing an aspiration pneumonia model, lung lobe grafts were evaluated using lung ultrasound and a local respiratory function measurement device. These results indicated lung ultrasound is the most helpful tool for functional evaluation after lung transplantation.

Academic Significance and Societal Importance of the Research Achievements

本研究の成果は移植可能なドナー肺葉の数を増加させることが期待できる.大型ドナーの肺葉を用いた小柄なレシピエント,特に小児への肺葉移植数が増加すれば,肺移植待機中の死亡を減少させることができる.また,レシピエントの急速な呼吸不全増悪に対する緊急回避的生体肺移植数を減じ, 健康な生体ドナーの肺提供手術が回避される.本研究はドナー肺が不足している本邦において特に必要とされるもので,その社会的意義は大きい.
