Research Abstract

①血中脂質メディエーターなどの測定と新規脂質メディエーターの同定：　この新学術領域研究の領域代表者である横溝岳彦教授の依頼を受け、久山町住民のサンプルを用いてスフィンゴシン１リン酸受容体（S1P1）遺伝子上にある34か所の一塩基多型（SNP）の測定を行った。いずれのSNPもアリル頻度が極めて低く、生活習慣病との関連を検討することはできなかった。
②対象集団の追跡調査およびデータベース構築：　2002年の久山町健診を受診した3298名の追跡調査を継続した。2012年までの10年間に発症した心血管病、認知症および死亡のイベントをデータベースに追加した。
③脂肪酸分画および1,25(OH)2ビタミンDと生活習慣病との関連：　昨年度までに検討した血清エイコサペンタエン酸(EPA)/アラキドン酸(AA)比と心血管病発症の関連について、その成果を英文誌（Atherosclerosis 2013; 231:261-267）に発表した。今年度の新たな研究として、2002年の健診を受診した40歳以上の久山町住民2417名を5年間追跡し、血清1,25(OH)2ビタミンD濃度と慢性腎臓病発症との関係を検討した。血清ビタミンD濃度で対象者を4群（4分位）に分けて検討すると、第4分位を基準とした慢性腎臓病発症のオッズ比（多変量調整後）は第1分位で1.90、第2分位で1.74と有意に上昇し、血清ビタミンDレベルの低下は慢性腎臓病発症の有意な独立した危険因子であった。さらに、血清ビタミンDレベルの低下に伴って、5年間の腎機能（推算糸球体濾過率）低下の程度がより急速であることが明らかとなった。以上の成果を英文誌（Circ J 2014; 78:732-737)に発表した。

Reason

久山町の一般住民を対象としたコホート研究において、脂質メディエーターのひとつである血清ビタミンD濃度と慢性腎臓病発症との関係を解析し、一定の知見を得た。さらにこの研究成果を論文にて発表することができた。

Strategy for Future Research Activity

引き続き、久山町の住民集団の追跡調査を継続し、データベースを更新する。その追跡調査の成績を用いて、脂質メディエーターおよびその関連遺伝子と生活習慣病発症との関係を検討する予定である。研究計画の変更や研究遂行上の問題点は特にない。
