毛髪による治療的薬物モニタリングの方法論の確立:服薬状況把握への応用

| Research Abstract |
昨年度の成果より、薬物が投薬量に応じて毛髪内へ取り込まれること、取り込まれた薬物は毛髪の中に保持され毛髪の生長に伴って毛根側から先端側へ移動することが判明した。しかし、毛髪の生長スピ-ドには個人差が存在し、又、毛髪には成長期と休止期があることなど問題点が明かとなった。そこで、日常診療で汎用され、短期間のみ原病に関係なく使用される可能性が大きい薬物で毛髪より検出され易い薬物を見いだすために多くの薬物のスクリ-ニングを行った。尿路・呼吸器・耳鼻科領域感染症、更に感冒に広く用いられる合成抗菌薬オフロキサシンに目標を絞り、その毛髪よりの抽出法・高速液体クロマトグラフィ-を用いた微量定量法を確立した。健常成人ボランティアにオフロキサシン製剤を用量を替えて投与し検討したところ、本剤を一錠服用しただけで毛髪から検出でき、しかも毛髪内濃度が投与量に比例することが判明した。次に、過去にオフロキサシンを服薬したことのある患者より毛髪を採取し、毛髪を毛根より1cmづつに切り分けてそれぞれのセグメントよりオフロキサシンを測定した。毛髪の生長を一月1cmと仮定すると、毛髪内での本剤の分布が過去の服薬歴とよく一致することが証明された。更に、過去一月以内に本剤を服用した患者より、毛髪を一月毎に服薬後三カ月まで採取し同様にオフロキサシンの毛髪内分布を検討すると、一月に1cmづつ先端側へ移動していくことが判明した。このことは毛髪内のオフロキサシン分布と本剤の服薬歴を調べて対比させると、その個人の毛髪の生長スピ-ド当該毛髪の生長サイクルを知ることが出来ることを示している。服薬状況把握への応用としては、毛髪内ニコチン濃度の測定法を確立し禁煙教室で禁煙を決意したボランティアで6カ月の禁煙教室期間中の喫煙状況と毛髪内のニコチン分布とを対比すると禁煙の経過とよく一致し喫煙或いは禁煙状況がよく把握できることが示された。
