小腸移植後の消化吸収能と消化管ホルモンの動態

Research Abstract	小腸移植は腸管自身の持つ強い抗原性による激しい拒絶反応と移植片が含有するリンパ組織のための移植片対宿主病の免疫学的問題を克服しなければならない.我々はこのような小腸移植の免疫的特性を基礎的面から研究してきた.該当年は免疫反応の軽減を目的として部分小腸を臨床応用のための基礎的研究を行った.まず腸短症候群の病態ならびに発育に必要な移植腸管の長さを検討し,つぎに消化吸収能等の栄養学的面からみて空腸または回腸のどちらかを移植したらよいかを検討した.1.全小腸摘除を行いラットにおける腸短症候群を検討した.幼弱ラット(生後2-4週)では小腸を全摘除すると静脈栄養等の補助なしでは1週間以内に死亡するが,8週以降のラットでは持続的低栄養を示して長期生存した.腸管切除により長期低栄養を示したラットは正球性低色素性貧血を示し,また免疫系臓器にも異常を来した。特に肝ではfocal hepatitisを生じていた.8週以降のラットを用いて終末回腸を約10cm移植すると正常に近い体重増加が観察され,部分小腸移植成功の可能性を示した.2.移植片に空腸と回腸をそれぞれ約35cm用いた上で,実験群を正常(無処置)群,空腸移植群,回腸移植群,腸短(小腸を全摘除)群に分け検討した.いずれの群も食事摂取量は同様であったが,腸短群は術後体重増加が著しく障害され,空腸移植群,回腸移植群はともに正常群と同様の体重増加を示した.排便成分の内の脂肪排拙量を測定すると空腸移植群は脂肪吸収が障害されていたが,回腸移植群は正常群と同様の結果を示した.胆汁中の胆汁酸量を測定すると空腸移植群で低下していたが,回腸移植群は正常群と同等であった.移植腸管内にオレイン酸負荷を行うと,胆汁流出量には差がなかったが,血清中リパーゼ値が回腸移植群で有意に高値を示した.更にリンパ管造影を行い移植後のリンパ管再疎通性を検討すると,術後3日目ではいまだ再疎通していないが,術後4週目には移植腸管からホストの胸管まで再開通していることが確認された.
