抗癌剤とキメラ抗体との複合体にる大腸癌手術後の肝転移再発防止の研究

Research Abstract

本研究の目的は、新しく開発された副作用の少ないキメラ抗体と抗癌剤ネオカルチノスタチンとの複合体を合成し、これを用いて大腸癌手術後に残存している微小転移巣を治療することで、肝転移再発を防止することである。はじめに、臨床第1相試験の後に採取された患者血清の免疫学的解析が進められた。そして、キメラ抗体との複合体では、ヒト抗マウス抗体の産生がマウス抗体の複合体を投与した場に比較して強く抑制されていることを明らかにした。この抑制は長期にわたって検討しても持続し、キメラ複合体のほうがより安全なことが示された。ただし、この際に検出されるヒト抗マウス抗体は、抗体のうち特に抗原性の強いFc部分に対するものではなく、マウスの構造が温存されている可変領域に対するものであった。すなわち、抗イディオタイプ抗体が産生されたものと考えられた。これが、抗体の癌細胞への結合にどのような影響を与えるのか、投与された複合体の血中動態をどのように変化させるのか、今後さらに検討する必要がある。
本研究で用いられてた抗体A7の認識抗原は細胞膜上に存在し、分子量約4万5千の糖蛋白質であることはわかっているものの、分子としての同定が未だ行われていない。この標的分子の解析が開始されたが、具体的な成果は出なかった。なお、臨床研究は新しいGCPが示されたので、これに学内の審査制度が対応してから認可されることになった。以上のように本研究の目的とするところを100%達成することはできなかったが、この新しい治療法の完成に向けて、確実な進展が得られたと考えている。
