| Research Abstract |
本年度は、高齢者介護サービス24時間化の前提となる受給者の生活行動リズムを中心に研究を行った。
1.携行型行動量計による痴呆高齢者の行動量リズムの測定
秋田県内N町にある老人保健施設に入所している52名の痴呆高齢者を対象に、行動量リズムの測定を1分間隔で7日間連続測定した。同時に対象者の医学的履歴、痴呆の程度(厚生省基準)、日常生活自立度(障害老人の厚生省基準)、要介護度認定(厚生吝基準)を行った。52名中23名(44%)に行動量のサーカディアンリズムの内的脱同調が認められた。行動量リズム異常は、昼夜境界消失型、位相後退型、振幅減少型、混合型の4つに分類することができた。夜明徘徊がもっとも多く認められるのは昼夜境界消失型であった。昼夜境界消失型では在宅ケアにおける夜間監視業務の必要性が高いと考えられるが、施設ケアにおいては他の型と比べてとくに夜間監視業務サービス量のニーズは高くなかった。位相後退型においては、入浴サービスがリズム異常を是正する可能性あることが新知見として得られた。痴呆高齢者においては、行動量リズム異常の頻度が高いことから、行動量リズム異常を考慮にいれた施設ケア、在宅ケアのサービス供給の決定が必要であることが明らかとなった。
2.地域リハビリテーションサービス受給者の生活行動リズムに関する研究
地域リハビリテーションサービスを受給している秋田市在住の24名および名取市在住の60名の脳卒中後遺症者を対象に、簡易生活リズム質問紙による生活リズム評価、生活体力測定を行い、日常生活動作龍力と生活リズム同調の関連性を調べた。簡易生活リズム質問紙による生活ハズム同調状態の把握は容易であり、かつ日常生活動作能力と生活リズム同調は高い関連性のあることが明らかにされた。さらに、生活リズム同調の重要性を受給者に認識させることは、健康教育の一環として有用であることが明らかになった。
