変性関節軟骨の再生における接着分子の発現とその再生過程

Research Abstract	今回我々は変形性膝関節症後の再生軟骨における成長因子の発現を軟骨細胞の増殖やコラーゲンの増生を促進する作用を持つIGF-1(insulin like growth factor)を用いて検討した.高位脛骨骨切り術後抜釘時に,実験の趣旨を良く説明し患者の承諾を得たうえで,大腿骨内側顆部の荷重部周辺より軟骨下骨を含む約5×5mmの検体を採取した.症例は16人24膝であった.抜釘までの平均観察期間は15ヶ月(8-28)で平均年齢は70歳(57-78)であった.変性軟骨と再生軟骨の評価は当科における評価法を用いた.軟骨下骨が露出する変性Grade4以上では軟骨の再生度が高い傾向が見られた.検体は約1週間各染色法に応じてそれぞれ4%PEA,10%中性ホルマリンにて固定後約1ヶ月EDTA(0.5M,pH7.2)にて脱灰を施行した.その後は通常の方法に従いパラフィン包埋後4〜6μmの連続切片を作成した.染色は抗IGF-1抗体と抗Type II collagen抗体を用いた免疫組織学的染色と,IGF-1のmRNA発現を観察する目的でジゴキシゲニン標識した合成オリゴプローブを用い非放射性にin situ hybridizationを施行した.IGF-1mRNAの発現は24例中21例に認められIGF-1の局所での発現が示唆された.抗IGF-1抗体を用いた免疫組織化学的染色では24例中21例に,またTypeII collagen染色では24例中21例に陽性反応が見られた.IGF-1mRNAの発現は浅層より若干中間層から深層にかけて濃染していた.TypeII collagenも中間層から深層にかけて染色され,相関が認められる.今後は引き続きラットもを用いて関節軟骨修復過程における接着分子CD44やICAM-1等の発現を中心に軟骨再生の基序を解析する予定である.
