| Research Abstract |
口腔癌の臨床病理像には国により地方により差がみられるが、以下の研究を始める第一歩として口腔癌の多数例についての地理病理学的研究を行う。
口腔癌の発生は外因性因子が作用して起こるが、まだその明確なものは同定されていない。今回の研究は口腔癌の発生を腫瘍ウイルスであるヒトパピローマウイルス(HPV)の面から、口腔内多発癌および口腔癌と喉頭癌の重複癌についてはloss of heterozygosity(LOH)を用いて検討した。
HPVには多種類あるので、HPV・DNAがロシア人・中国人・沖縄住民の口腔癌でどのような頻度で、またどのような型が多いかを検討し、すでにデータが得られている私達の日本人の口腔癌に関するデータと比較して口腔癌の起こり方を検索した。
口腔癌は前癌性病変である白板症などの病変から発生するときは多発することが知られており、field cancerizationの概念として提唱されている。これは外来性因子が口腔粘膜を広い範囲にわたって癌発生の準備状態にするためと考えられる。日本人の口腔癌を用いてマイクロディセクション法によるLOHを用いて、多発癌を起こす機序を癌抑制遺伝子の面から検索した。
口腔癌と喉頭癌が重複して発生することがよく知られているが、その機序はよく分かっていない。LOHを用い、日本人とロシア人で比較しながら検索している。
