| Research Abstract |
各種固形癌(胃癌、食道癌、大腸癌、肺癌、膵臓癌、肝癌、胆道癌、乳癌)の微小転移検出を目的としたMultiple-marker RT-PCR assay法を確立した。癌患者の手術時の郭清リンパ節あるいは、血液サンプルを用いてこの遺伝子診断による微小転移検出を行いその有用性を証明した。(Int.J Cancer,2001,Cancer,2000,Clin.Cancer Res.,2000,Int.J.Oncol.,2000,Digest.Dis.Sci.,2000)遺伝子診断における、至適サンプリング法の確立を行い報告した。(Cancer letter,2000)
遺伝子診断の臨床応用のために、リアルタイムモニタリングシステムによる遺伝子マーカーの定量的検出法と、術中診断を可能にするためのキャピラリーPCRを用いた迅速診断法を確立した。このシステムを用いた癌治療への臨床応用として、術中迅速遺伝子診断法によるリンパ節微小転移診断に基づく、食道癌のリンパ節郭清範囲の決定の有用性の検討を行った。従来、頸部リンパ節転移のセンチネルノードとして、迅速病理診断を行っていた反回神経周囲リンパ節の迅速遺伝子診断により、リンパ節の個数にして、22個/68個(22%)、症例数にして、12例/31例(39%)が新たに転移陽性となった。実際に、これらの12症例のうち3例に頸部転移、あるいは再発を認め、残りの19例には認めなかった。これについては学会発表を行うとともに現在論文作成中である。又、高度進行胃癌を対象とした、化学療法の効果判定として、laparoscopy下に採取した腹腔洗浄液を用いた、癌細胞の定量的遺伝子診断法を確立し、この有効性を学会発表、論文にまとめる予定である。
