| Research Abstract |
臨床研究:
まず、レビー小体型痴呆(DLB)の疫学について、剖検例での検討を行い、文献報告と比較研究した。DLBは痴呆例の18%で、アルツハイマー型痴呆(ATD:43%)、血管性痴呆(23%)に次いで三番目に多く、DLBは二番目に多いという外国の文献と若干の違いがあった。また、剖検により診断が確定した症例の検討で、DLBでもパーキンソン病と同様に自律神経症状が目立つことを報告した。治療の研究としては、DLBではATD以上に脳のacetylcholineが低下していることから、ATDの治療薬として承認されているdonepezilの効果をみるために、三施設で治験を試み、13例のデータを現在分析中であり、近いうちに論文発表の予定である。
病理学的研究:
まず、1978年に小阪が報告した皮質型レビー小体の成長・消退過程を免疫組織学的に再検討し、以前の所見を再確認し、レビー小体が神経細胞死と密接に関連することを明らかにした。また、免疫組織学的にDLB脳では神経細胞のRNA oxidationが目立ち、またサイトカインやmetric oxidesynthaseの発現がみられることを明らかにした。また、DLBの大脳皮質や海馬領域ではtyrosine hydroxylase陽性の神経細胞が変性しやすいことを明らかにし、報告した。
