Research Abstract	胃切除後の患者で、食欲が低下し、体重が減少する原因として切除に伴う消化吸収障害が考えられている。近年同定されたペプチドであるグレリンは主に胃から分泌され、食欲亢進作用を示すことから、この病態に関与している可能性が考えられる。今回我々は、胃切除後の症例に対してグレリンの血中動態をフォローし、体重調節病態におけるグレリンの関与を調査した。対象は、胃全摘、亜全摘症例の計37例。胃切除症例より、インフォームドコンセントを得た後に、ワンポイントで血漿、血清を採取し、グレリン(C末端)、IGF-1、GH、レプチンの測定を行った。さらに、16症例については術前、術後二週間、一ヶ月のBMIを測定し、体重の推移とグレリン、及びその関連因子との相関を観察し、統計解析した。その結果、ほとんどの症例では、術前に比して、手術後二週間のグレリン値は有意に低下していた(117.5 vs72.6fmol/ml)。術式で比較した場合、全摘出群のほうが部分切除群に比して、グレリンは低値であった(58.2 vs84.2fmol/l)。一ヵ月後のグレリン値は引き続き低下する群と上昇に転ずる2群に分けられ、代償性のグレリンの分泌が見られる群と見られない群があることがはじめて示唆された。現在、それぞれの群における摘出胃組織の免疫染色を行っている。さらに、血中レプチン、IGF-1のレベルはグレリンの動向と一致して推移する傾向が見られた。胃切除後の体重の減少、その後の回復過程において、グレリンが関与している可能性が示唆されたが、今後更なる長期のフォローとグレリン分泌代償機構の解明が必要であると考えられる。
