閉経の筋肉量、体脂肪量、体脂肪分布に及ぼす影響とホルモン補充療法の予防効果

Research Abstract

閉経前、閉経後女性における体組成が骨密度に及ぼす効果の差
1.背景・目的
本研究では、閉経前および閉経後女性における体脂肪成分と除体脂肪成分が骨密度(BMD)に及ぼす効果に差があるか否かを検討することを目的とした。
2.対象・方法
360人の閉経前、193人の閉経後日本人女性を対象とし、年齢、身長、閉経からの年数を記録した。体脂肪量と除体脂肪量はDXAを用いた全身スキャンにより測定した。縦軸(第2-4腰椎、骨盤、両側下肢、全身)のBMDと水平軸(上肢)のBMDもDXAにより測定した。
3.結果
閉経前女性では除体脂肪量は各部位のBMDと独立して関連があり、偏相関指数は左上肢:0.417、右上肢:0.430、右下肢:0.412、全身:0.377(p<0.001)であった。しかし、体脂肪量との相関が認められたのは骨盤BMDのみであった(0.187、p<0.01)。閉経後女性では、体脂肪量は各部位のBMDと独立して相関しており、偏相関指数は左上肢:0.248(p<0.01)、第2-4腰椎:0.188(p<0.05)、骨盤:0.263(p<0.01)、左下肢:0.228(p<0.01)、右下肢:0.319(p<0.001)、全身:0.188(p<0.01)(表)であった。しかし、除体脂肪量と相関があったのは左上肢:0.175、右上肢:0.217、左下肢:0.210(p<0.05)のBMDのみであった。
4.結論
閉経前女性では除体脂肪量がBMDを規定する重要な因子であるが、閉経後女性では体脂肪量が重要な因子となることが示された。
