Outline of Annual Research Achievements

前回までに捉えられていた抗好中球細胞質抗体(Antineutrophil cytoplasmic antibody, ANCA)関連血管炎患者の血清においてSEMA4Dが高値である点、およびSEMA4D-Plexin-B2依存性に誘導される好中球におけるNeutrophil extracellular traps (NET)の抑制作用について、機能解析を行った。好中球上に発現する膜型SEMA4Dは、蛋白質分解酵素ADAM17により切断され、遊離型になることが分かった。また、血管内皮細胞上のPlexin-B2が好中球の表面に発現するSEMA4Dと結合した際、NET形成に必須とされる好中球の活性酸素(ROS)が、細胞内タンパク質であるRac1の非活性化を介して負に制御されることが明らかとなった。さらに、リコンビナントPlexin-B2タンパク質を用いて、ANCA関連血管炎患者由来のIgGにより活性化されたヒト好中球を処理したところ、NETの放出を有意に抑制することができた。
以上より、AAV発症のメカニズムとして、健常者では血管に発現するPlexin-B2と、好中球に発現するSEMA4Dが結合し、好中球の異常な活性化にブレーキをかけているが、AAV患者ではこのバランスが破綻し、ADAM17の働きによって好中球のSEMA4Dが切断されることで好中球のブレーキがはずれ、不適切に活性化してしまっているという機序が考えられた。
