| Outline of Annual Research Achievements |
2015年5-8月に高知県土佐町在住高齢者と有料老人ホーム 「ライフ・イン京都」在住高齢者を対象とした高齢者総合機能評価を行なった。事前指示書作成者はそれぞれ回答者の15.6％（N=815）と 60.3％（N=239）であった。事前指示書作成者は年齢が高く、Visual Analogue Scale（VAS）で測定した主観的健康感が有意に低い（P=0.004）ことが判明した。一方で男女差やGeriatric Depression Score で測定した抑うつ度に差はなかった。VASで測定したQOLと、健康関連QOL測定法としてひろく使用されている SF-8 の関連についての報告はこれまでにない。有料老人ホーム「ライフ･イン京都」在住高齢者を対象とし、SF-8の各因子とVASで測定した主観的健康感、幸福感の関連をSpearman順位相関係数で検討した。SF-8の全体的健康感（General Health）とVASで測定した主観的健康感は有意に相関していた。（ρ＝0.53）VASで測定した主観的幸福感はSF-8の心の健康（Mental Health）ともっとも強く相関していた（ρ＝0.61）VASにより測定した主観的健康感はSF-8の全体的健康感と比較的強い相関があり、健康関連QOL測定に有用である。主観的幸福感（VAS）の構成概念は多面的であるがSF-8 の身体機能関連の項目よりも心の健康がより強く関連していた。

高知県土佐町在住高齢者と有料老人ホーム 「ライフ・イン京都」在住高齢者を対象とした高齢者総合機能評価をおこない、事前指示の作成状況を明らかにするとともに、認知・行動機能検査、抑うつ評価を予定通り実施した。また、ライフ・イン京都居住者を対象とした事前指示書に関する広報活動もおこない、ほぼ計画通りに進捗している。

| Strategy for Future Research Activity |
ひきつづき両地域での総合機能評価健診をおこなって、事前指示書の作成状況を明らかにするとともに、死亡者を対象とした終末期ケアの実態調査をおこなう。
また、適宜、英国・レスター大学、インドネシア・チェンデラワシ大学、スペイン・アンダルシア公衆衛生学院の研究者らと連携して高齢者終末期ケアの実態に関する情報収集を行う。

27年度は、H24-26年度に行った基盤研究（C)高齢者終末期ケアに関する事前指示の縦断的検討-総合機能評価の視点からーの延長期間であった。そのため、予定していた旅費と人件費のおおくをそこから支出したため、次年度使用額が生じた。
