【背景】血清トリグリセライド(TG)は、グリセロールに3つの脂肪酸がエステル結合している。しかし、血中には脂肪酸が結合していない遊離グリセロール(FG)、脂肪酸が1つ結合したモノアシルグリセロール（MAG）、2つ結合したジアシルグリセロール（DAG）も少量は存在する。日本ではFGを消去してからMAG, DAG, TGをリパーゼで水解し、分解されて放出されるグリセロール量でTGを測定する方法（消去法）が普及しているが、欧米ではFGを消去せずに測定する方法（未消去法）が普及している。本研究では、過年度までは未消去法のTG値から消去法のTG値を減じてFC濃度を計算で求めていた。しかし、計算法の正確度については検証できていない。
【目的】ガスクロマトグラフィー質量分析法（GC/MS法）によるFGの測定系を確立する、GC/MS法で測定したFGとTG値との間に関係があるか明らかにする、という2点を目的とした。
【対象および方法】成人健常ボランティア20名を対象とし、血清を分離した。血清にグリセロールの内部標準物質を添加し、クロロホルム/メタノールで脂質分画を抽出したのち濃縮遠心機で乾固させた。脂質をトリメチルシリル化（TMS）化し、GC/MSで定量した。
【結果および考察】確立したGC/MS法によるFG値は、CV=5.8%と良好な同時再現性を示した。また、240μmol/L（TG換算で21.3 mg/dL）まで良好な直線性を認めた。健常人のFGは、52～102μmol/L（TG換算で4.6～9.0 mg/dL）の範囲だった。消去法で測定したTGとの間には、全く相関を認めなかった。これは、血清FG値が脂肪組織由来であり、リポ蛋白由来でないことを示唆する。【結論】FGをGC/MS法で定量する測定系を確立した。今後は、未消去法と消去法の差でFGを測定する簡易法がどれほど正確かを検証する必要がある。
