Outline of Annual Research Achievements

心筋虚血指標の冠動脈心筋血流予備量比(FFRmyo)は、平均冠動脈内圧を用いており、拡張期冠動脈内圧のみならず収縮期冠動脈内圧を含めた形で計測されているが、冠内圧を冠血流の代用とできるのは拡張期冠血流ー内圧関係から拡張期のみである。コンピューター技術が進歩した現在では「拡張期のFFRmyo:d-FFR」を容易に計測でき、FFRmyoより精度の良い心筋虚血の指標を得ることができる可能性がある。この仮説のもとに、d-FFR計測ソフトウエアを開発し、心筋虚血の診断の精度の向上を検討した。この間に、FFRmyo以外に様々な安静時心筋虚血指標が提案され、負荷なしで簡便に得られる指標で、FFRmyoとの比較では非劣勢が示され、日常臨床に広まりつつある。新しい指標は、境界領域の中等度狭窄での診断精度に限界が指摘されている。また、本研究のd-FFRに関する報告はなく、d-FFRが種々の心筋虚血診断のゴールドスタンダードになる可能性が期待される。
① 拡張期心筋血流予備量比(d-FFR)の基本ソフトウエアの精度をさらに向上させるため、動物実験および日常臨床例で前向きに記録した冠内圧記録・心筋血流予備量比(FFRmyo)記録を用いてd-FFR計測精度を追加検討した。
② 従来のFFRmyoとd-FFRおよび心筋シンチグラフィーとの比較症例数を増やして解析検討中である。また、自施設の倫理委員会で承認を得て、心筋シンチをゴールドスタンダードとしてd-FFR・FFRmyoの診断精度を研究する医師主導型の多施設共同研究(DIASTOLE study)を推進し、450例の登録予定(有効解析症例413例)のうち409例の登録が2019年4月末時点で終了しており、残り4例の登録で研究登録を終了することができ、解析を行う予定である。解析結果は本年度中に学会報告し科学雑誌に投稿する。
