Outline of Annual Research Achievements

全肺洗浄(WLL)において除去される自己抗体量、肺が産生するGM-CSF量、肺胞マクロファージによる自己抗体の吸収分解の3因子からなる数学モデルを構築した。15例-30回のWLLの毎回の排液中の自己抗体濃度、全IgG濃度、自己抗体が全IgG濃度に占める割合はすでに測定し、残りの検体を保存している。
第1段階では、WLL中の自己抗体濃度を推測する数学モデル式を上記排液に適用し、自己抗体濃度、IgG濃度の実測値と肺が産生するGM-CSF量から、このモデルを検証する。第2段階として第1段階で検証した数学モデルを発展させ、WLL後に肺にある自己抗体が増加するか、一定あるいは収束するかを予測する数学モデルを勘案した。
29年度の課題は、自己抗体と免疫複合体を形成している内因性GM-CSF濃度を正確に測定することであった。これに対し、複合体をポリクローナル抗体でトラップし、酸で抽出することによりGM-CSFを遊離させ、アルカリフォスファターゼ標識抗GM-CSFモノクローナル抗体と結合させたのち、別のモノクローナル抗体により捕捉するサンドイッチELISAを行う。検出は、感度のよい化学発光法を用いた。この方法によるGM-CSFの測定感度は、3.125pg/ml であった。
