Outline of Annual Research Achievements

初年度にあたる平成28年度は実施計画に基づいて以下の研究を実施した。
1)参加20医療機関における臨床検体の収集：コンソーシアムに参加する20医療機関において、新規手術症例に対する正常組織および腫瘍組織の検体収集を進めた。さらに、解析対象30疾患を中心に既存検体の保有数確認および臨床情報の整理を行った。2)コンソーシアム事務局を中心としたネットワーク構築と試料管理：コンソーシアム事務局が中心となり参加機関における倫理審査承認・更新を進めた。また平成28年7月16日と平成29年1月21日に班会議を実施し、試料収集や研究進捗状況の報告、新規研究・共同研究の提案などを審議した。また一部の組織型について各医療機関から事務局へ計386検体の試料（腫瘍組織:227、正常組織:159）を送付し、凍結検体からの病理切片の作成と核酸の抽出・評価を実施した。3)中央病理診断：2)で収集した凍結腫瘍検体73検体についてHE染色組織標本を作成し、腫瘍細胞の含有率の測定を行った。また各施設から診断時に使用した病理標本を事務局経由で収集の上、中央病理診断施設にて診断の確定と解析対象症例の選択を実施した。4)骨軟部腫瘍に対する腫瘍特異的融合遺伝子の探索：2)によって精製されたRNAを用いて腫瘍組織138検体についてRNAシークエンスを実施し、高頻度に同定された融合遺伝子や治療標的分子については、同一検体での検証、独立検体での再現性の確認を進めた。5) 腫瘍特異的融合遺伝子の機能解析：脱分化型脂肪肉腫について他施設の解析結果も用いたメタ解析を現在実施した。さらに臨床情報と突合した解析を実施し予後予測因子などについて検討をすすめた。6) 情報公開と共同研究の推進：整形外科関連・がん関連の学会などでコンソーシアムの活動を周知した。国内の医療機関のリクルートを進め、新規に2医療機関の参加が得られた。

Reason

参加20医療機関における臨床検体の新規収集については、各医療機関における新規症例の受診状況による部分が大きいが、全体としてみると骨軟部腫瘍の中でも特に希少な組織型も含めて収集が進んでいる。事務局を中心とした各医療機関・解析機関との連携として、年2回の班会議のほか個別に研究代表者、解析機関、事務局の間で打ち合わせも実施し、コンソーシアム内のネットワーク充実を図った。
ゲノム解析の対象とした8組織型（明細胞肉腫、胞巣状軟部肉腫、二次性軟骨肉腫、骨外性骨肉腫、デスモイド、腱滑膜巨細胞腫、fibroma of tendon sheeth、desmoplastic fibroblastoma）について227症例の腫瘍組織検体を収集した。このうち正常組織と共に収集できた症例を中心に計100例を超える腫瘍組織を用いて中央病理診断機関での組織学的評価を進めると共にRNAシークエンス解析を完了した。さらにデータ解析を進め、2次解析として融合遺伝子解析、遺伝子発現解析を行った。一部の組織型においては複数検体で共通の融合遺伝子を同定しており、これに対して現在、検証解析を進めると共に機能解析を進めている。また、先行して解析を進めている脱分化型脂肪肉腫においてはメタ解析として121症例の解析を実施中であり、予後との関連を示す遺伝子異常も同定した。約30%の頻度で見られる新規融合遺伝子も明らかとしている。コンソーシアムの活動を周知するため、国際学会(The 11th Meeting of The Asia Pacific Musculoskeletal Tumour Society、2016 Connective Tissue Oncology Society Annual Meeting)・国内学会（第49回日本整形外科学会骨軟部腫瘍学術集会、第75回日本癌学会学術総会）での報告を行った。

Strategy for Future Research Activity

これまでのところ当初目標に向けて順調に研究が進展しており、引き続き当初計画に基づいて研究を推進して行く予定である。
1) 参加20医療機関における臨床検体の収集：検体収集については前述の如く各医療機関で順次進められており、今後も新規の解析対象組織について臨床情報の整理および検体の収集を継続する。2) コンソーシアム事務局を中心としたネットワーク構築と試料管理：既存参加施設においては個人情報保護法改正に対する倫理審査対応をすすめると共に、新規参加施設においても同じく改正個人情報保護法に対応した倫理審査の承認を目指す。コンソーシアム内のネットワークはメールベースでの定期連絡、年2回の班会議を継続すると共に、必要に応じた関係者間での個別打ち合わせを実施し、研究を推進する。3) 中央病理診断：新規解析対象組織型について事務局への収集を進め、凍結組織からの組織標本の作成、核酸抽出を行う。凍結組織標本についてはこれまでと同様、腫瘍細胞含有率の確認を行うと共に、各医療機関から診断時の病理資料を収集し中央病理機関での病理診断の検証を行う。4) 骨軟部腫瘍に対する腫瘍特異的融合遺伝子の探索：中央病理診断で解析対象検体として確認できた症例について、引き続きRNAシークエンスによる融合遺伝子解析・発現解析を進める。5) 腫瘍特異的融合遺伝子の機能解析：これまでの解析で高頻度に認められた融合遺伝子についてその機能解析を行う。機能解析においてドライバー変異としての可能性が示唆されたものについては、薬剤スクリーニングなどを実施し治療標的としての可能性について検討する。また、組織型ごとの個別の解析のみならず組織型横断的な発現解析も実施し、トランスクリプトームデータのカタログ作成を進める。6) 情報公開と共同研究の推進：引き続きコンソーシアムの活動を学会等で周知し、医療機関のリクルートを進める。
