申請者らはウイルス排除後にも依然発癌リスクが存在することを明らかとしてきた(Hepatology 2010, 2013)。今回、半導体シークエンサーを用いて肝細胞癌関連遺伝子プロファイルを網羅的に解析し、HCV排除（SVR）の有無および核酸アナログ(NUCs)によるHBV複製制御の有無、およびHBV排除後における発癌要因の差異について検討した。対象は肝切除をした197例。HCV陽性肝癌はTP53 変異は少ないが(P= 0.002)、TERT promoter変異が多く(P=0.003)、AXIN1やRB1 などWNT signaling pathwayの遺伝子変異やARID1A、ARID2などのクロマチン修飾に関わる遺伝子変異を認めたがSVRの有無による遺伝子変異の頻度の差は認めなかった。HBV持続感染肝癌はTP53変異が比較的高頻度で、HBV integrationは93%で検出されヒト側遺伝子ではTERT、MLL4、MYO7Aが多かった。NUCs内服の有無でTERT領域も含めHBV integrationの頻度に差はなかったがTERT promoter変異はNUCs内服群で内服していない群と比較して低下しており、HBV DNA検出感度未満の症例で有意に少なかった(0% versus 40%, p=0.02)。一方NBNC肝癌はTERT promoter 64.6%, CTNNB1 33.3%, TP53 41.0%で、そのうちHBV既往感染例の遺伝子変異はTERT promoter71.4%、CTNNB138.0%、TP53 71.4%であった。HBV integrationは12%で検出され、HCV抗体陰性、若年発症、正常肝からの発癌という特徴を有していた。
