術前の良悪性の判断が困難な膵管内乳頭粘液性腫瘍(IPMN)に対し、申請者らは３施設における1996 年～2014 年までに術前にEUS/CT/MRI を用いて詳細に画像診断を行ったIPMN 切除症例466例の検討においてロジスティック回帰分析から抽出された汎用性の高い3因子 (病型、結節径,嚢胞径)を用いてオッズ比より悪性度スコア癌予測モデルを作成した。さらに、 全国8施設の664例の切除症例を用いて、その外的妥当性を評価した。466例のモデルでは、258 (55%)が悪性のIPMNであった。多変量解析にて壁在結節径、主膵管径、嚢胞径の3因子が有意な癌予測因子であり、この3因子による癌予測モデルのAUCは0.763であった。external validationでは、351例(53%)が悪性であった。癌予測モデルを使ったexternal validationのAUCは、0.725と良好であった。
