Outline of Annual Research Achievements

半月板は膝関節において、大腿骨顆部と脛骨プラトーの間の内外側に1つずつある線維軟骨組織であり、荷重時等に関節軟骨や軟骨下骨に加わる応力を分散させるshock absorberとして極めて重要な機能を担っている一方で、一度損傷すると自己治癒能力が極めて乏しい組織である。半月板損傷は若年者のスポーツや外傷による損傷と、中高年者の加齢変性による損傷に大別されるが、後者は50歳以上の一般集団の30%弱に認められるとの報告 (Englund M; N Engl J Med 2008)があるなど非常に一般的な加齢に伴う変性疾患であり、また変形性膝関節症の発症に繋がる前段階の病態であることも知られている (Badlani JT; Am J Sports Med 2013)。半月板損傷に対する治療には、主には半月板切除術と半月板縫合術とがあるが、加齢変性に伴う中高年者の半月板損傷に対しては半月板縫合術の術後成績は芳しくないため、損傷部位の切除が行われることが多い。iPS細胞由来の半月板組織が臨床現場で使用できるようになれば、現状では多くの場合切除しか有効な方法が存在しない本邦の半月板損傷治療において、切除後に新たな半月板組織を移植するという治療選択肢が生まれ、半月板の機能不全により生じる膝関節軟骨損傷や変形性膝関節症の発症予防へと繋げることが可能となる。
現在はマウス膝関節において半月板の切除を行い、当研究室で独自に確立したiPS細胞の分化誘導法を用いて誘導した軟骨前駆細胞を膝関節内に注射、半月板切除部位への生着性等の評価を行っている段階である。

ラットembryoの膝関節を用いた解析については、飼育設備が依然として整わず、また予算等の関係から次年度以降に見送ることとした。

Strategy for Future Research Activity

半月板切除を行ったマウス膝関節に対する、iPS細胞由来の軟骨前駆細胞の半月板切除部位への生着性等の評価を行っていく予定である。

Causes of Carryover

ラットembryoを用いた解析を次年度以降に見送ったため。今後は、in vivoの実験は、主にマウスを用いて解析を行っていく計画である。
