Outline of Annual Research Achievements

本研究の目的は、1)乳房の超高速造影ダイナミックMRIの画質改善、撮像・解析法の確立、および2)乳腺腫瘍血流・血管評価にもとづく診断や治療効果判定・予測法の確立である。
今年は最終年度として、治療効果評価への応用に向けた検討を開始、学会発表を行った。超高速造影ダイナミックMRIで得られる画像は、背景の非特異的な造影効果がみられるより前に、血流豊富な病変を描出でき、同様のことは血流が残存するViableな残存腫瘍にも見られる。逆に線維化を主体とした治療後のScar は造影されるが緩徐である。従来は造影の程度をkinetic curve を用いて評価していたが、超高速造影ダイナミックMRIの投影像(MIP)を用いることで数秒で標的病変の同定が可能であり、浸潤部のサイズ予測もより正確であることを学会にて発表、論文にまとめている。腫瘍周囲の血管の構築の複雑性を定量化する手法についても、多くの例で検討を重ね、増殖のマーカーであるKi-67index との相関が示された。国際学会にて発表の後、論文化に取り組んでいる。
血流は、乳癌のSubtypeによって異なることがわかってきた。超高速造影ダイナミックMRIから得られるKinetic parameterと拡散強調画像の組み合わせは良悪診断に有用であることは既に報告したが、浸潤癌のサブタイプ分類にも有用であることを我々は国際学会で発表した、特に、増殖能が高く、遺伝子変異との関連も深いTriple negative subtypeは、血流の豊富さがKinetic parameterに反映される。これと形態情報の組み合わせがTriple negative subtype分類に役立つ。これらについてもさらにSubtype 毎の検討を重ねている。
以上のテーマについて積極的に学会発表・口演を行い、研究成果の臨床への普及にも努めた。
