Outline of Final Research Achievements	A nationwide survey of local governments on the implementation and necessity of health checkups for 5-year-old children revealed that 14% of local governments have implemented health checkups for 5-year-old children. The purpose of the implementation is that the period from 3 years old to preschool age is a time when children do not have opportunities to receive health checkups, there are children with suspected developmental disorders during this period, and there are families with problems in childcare, which need to be addressed. In a survey of public health nurses in one municipality, 37.3% of the public health nurses indicated that they alone can grasp the growth and development of a 5-year-old child. In an information exchange with the University of Sydney, which has extensive experience in working with children with developmental disabilities, it was agreed that the first screening should certainly be conducted by nurses.

Academic Significance and Societal Importance of the Research Achievements	５歳健康診査の必要性が明らかとなり課題が示された。マンパワーと予算の確保を検討すること、実施方法の検討が急務である。マンパワーにおいては特に自治体の医師会の協力を得ることが必要である。実施方法については５歳児の評価の方法が各自治体によってまちまちであることから統一した評価方法を確立することが必要である。また関わっている専門職も多岐にわたっており、個別相談のみならず集団行動の観察を取り入れている自治体もあった。５歳児健康診査は１９８５年に川崎市で実施されたのが最も古く以後増加している。特に2,000年からの10年間で大きく増加していた。3歳児健康診査以降の切れ目ない支援の充実が必要である。
