Research Abstract

口腔粘膜癌の発生には、飲酒、喫煙や嗜好品等の食生活習慣が大きな要因となっていることは周知のところである。口腔がんの発生がとくに高率なアジア地域では、その発生要因として噛みタバコの習慣があげられる。噛みタバコ習慣は台湾から東南アジア諸国さらにスリランカ・インドの南アジアを経て、アラビア半島から東アフリカまで広がっており、同地域の口腔がんの高頻度発生とよく対応している。一方わが国では、近年口腔がんは増加傾向にあるが、噛みタバコの習慣はないが、増加の背景となる要因は特定できていない。このように社会、地域、民族、習慣等で口腔がんの発生機構は共通であるのか特異的なものかは不明である。そこで、噛みタバコによる口腔がんとわが国の口腔がんの発生背景を最新の分子病理学的技法で解析するために本研究課題を計画した。第一に、噛みタバコ習慣の広がるアジアからアフリカまでの地域における口腔癌の発生状況を正確に把握し、第二には、噛みタバコによる異型上皮と上皮内癌の病理組織学的特性を、上記各地域の口腔粘膜生検材料をもちいて細胞外基質ECM分子をはじめ種々の分子の発現動態を病変進行との関連で解明したい。第三は、噛みタバコ関連口腔がんと同習慣の全く存在しないわが国の口腔がんを比較し、その前癌病変から浸潤癌までの過程におけるがん関連遺伝子をはじめとする種々の形質発現のプロフィールをスクリーニングする予定である。
