Outline of Annual Research Achievements

わが国における最大の死因であるがんの原因や特性には遺伝子異常が深く関わっている。近年の科学技術の進歩によって、さまざまながんにおいてその発症や悪性度に関わる遺伝子異常の全体像が明らかにされてきた。しかしながら、その一方で、がんにおいて認められる遺伝子異常がすでにがん発症前から獲得されることが明らかにされ、それらが初期のがんの発生過程にどうかかわるかについてはいまだ不明である。さらには、その後に複数の遺伝子異常が積み重なることによってがんが高度に多様な細胞から構成され、他の臓器に浸潤や転移し、また臨床経過中に再発が惹起される過程の分子メカニズムについては、なお多くが不明である。これらを理解するためには、がんの起源である正常組織における微小なクローンに獲得される遺伝子異常を検出すること、遺伝子異常やその組み合わせと細胞のフェノタイプとの関係性を検討することが必要である。その際には通常では発見できないようなゲノムの異常構造などについて全貌を解明することが必須となる。これらの未解決の課題に対して、先端技術による微小なクローンの単離と全ゲノムシーケンス、単一細胞シーケンス、オルガノイド培養技術、遺伝子異常を導入したマウスモデルの解析、大規模コホートでの遺伝子異常と表現型の関係性の解析等を駆使して、包括的な探求を行った。その結果、骨髄異形成症候群（MDS）の多数例での解析により、TP53遺伝子が両アリル異常となっている状態が非常に重要な予後不良因子であることを明らかにした。また、MDSにおいて高頻度に遺伝子異常が共存するRUNX1とSTAG2が、協調的にクロマチンループ形成を阻害することでMDSの発症に関与していることを明らかにした。さらに乳腺や膵臓等での微少組織採取によるシーケンス解析やオルガノイド培養の技術を最適化し、さらなる解析を進めている。

Current Status of Research Progress

Reason

大腸癌の初期発生の解明では、前がん状態では選択される変異は大腸癌ではほとんど認めず、がんになりがたい遺伝子異常を同定した。これらは初期発生の解析にも関わらず、がんの治療標的候補にもつながりうる、想定した以上の非常に重要な知見である。
また、クローン造血の解析では、当初想定していなかった大規模な健常者サンプルと臨床情報が得られ、さらに大規模のSNPアレイデータの利用により、最新の超高感度コピー数解析を行うことが可能となった。その結果、クローン造血における遺伝子変異とコピー数解析の両者を同時に施行することが、造血器腫瘍および非腫瘍性疾患のリスク増大に重要なことが判明した。
さらに、当初は細胞レベルでの遺伝子変異と遺伝子発現の解析を単一細胞由来コロニーで解析する予定であったが、この解析では重要なサイトカインを添加した培養系での解析となるため、培養の影響が無視できない可能性が考えられた。我々の開発していた単一細胞での変異と発現同時解析技術の最適化が進み、培養を経ること無く、単一細胞レベルで遺伝子変異と遺伝子発現の同時解析が可能となった。この手法をクローン造血で用いることにより、変異を有する小クローンの遺伝子発現形質を解析可能となり、さらに、変異を有する症例の変異を有しない細胞の解析も可能となり、想定していなかった骨髄環境の変化を観測することができた。
MDSの解析では、我々が長年取り組んできたゲノム解析におけるコピー数異常の高度な解析技術を用いた大規模コホートの解析において、予後不良因子としてよく知られているTP53変異は複数のアリルで異常を来した場合に、強力な予後不良因子となることを明らかにした。治療方針の決定に影響する重要な知見を得ることができた。
これらの当初の期待以上の成果が得られている項目に加えて、他の項目でも順調に研究の進展が得られている。

Strategy for Future Research Activity

当初計画されたとおり、がんにおいて高頻度に認められるドライバー変異が獲得される時期およびそれらの組み合わせや蓄積を、臨床経過と関連させて解析することにより、それらが初期のがんの発生過程にどうかかわるか、また、がんが高度に多様な細胞から構成され、他の臓器に浸潤や転移し、また臨床経過中に再発が惹起される過程の分子メカニズムについて明らかにしていく。この研究では、がんにおいて高頻度に認められるドライバー変異は、すでにがん発症前から獲得されていることのみならず、がんにおけるそれらの検出頻度は正常組織とパターンが異なっており、遺伝子によっては、正常組織に認められるドライバー変異ががん組織で認められるよりも高頻度であることを見出しており、この現象が生じるメカニズムおよび、初期のがんの発生過程との関連を検討し、食道がん、大腸がん、造血器腫瘍において、がんおよび正常組織における遺伝子異常の検出、発症初期のメカニズムの解明、新たな病態の解明など、成果が得られた。その際に、がんの起源である正常組織における遺伝子異常を検出すること、遺伝子異常やその組み合わせと細胞のフェノタイプとの関係性を検討することを可能とした、ごく少量のサンプルからさまざまなシーケンス解析を行う独自の技術をさらに多くのがんに適用し、これまで未知のゲノムの異常を解明することを試みるともに、大規模コホートでの遺伝子異常と表現型の解析や、胚細胞遺伝子変異についても解析を行う。現在、研究が予定されているがんとして、乳がん、胃がん、前立腺がん、大腸がん、脳腫瘍、尿路上皮癌、造血器腫瘍が含まれる。これまで本研究は予定以上の成果をあげており計画の変更あるいは問題点などは認められない。
