Outline of Annual Research Achievements

わが国における最大の死因であるがんの原因や特性には遺伝子異常が深く関わっている。近年の科学技術の進歩によって、さまざまながんの発症や悪性度に関わる遺伝子異常の全体像が明らかにされてきた。しかしながら、がんに認められる遺伝子異常がすでにがん発症前に獲得されることが明らかにされ、それらが初期のがんの発生過程にどうかかわるかについてはいまだ不明である。さらには、その後に複数の遺伝子異常が積み重なることによってがんが高度に多様な細胞から構成され、他の臓器に浸潤や転移し、また臨床経過中に再発が惹起される過程の分子メカニズムについては、多くが不明である。これらを理解するためには、がんの起源である正常組織における微小なクローンに獲得される遺伝子異常を検出すること、遺伝子異常やその組み合わせと細胞のフェノタイプとの関係性を検討することが必要である。その際には通常では発見できないゲノムの異常構造などについて全貌を解明することが必須となる。これらの未解決の課題に対して、先端技術による微小なクローンの単離と全ゲノムシーケンス、単一細胞シーケンス、オルガノイド培養技術、遺伝子異常を導入したマウスモデルの解析、大規模コホートでの遺伝子異常と表現型の関係性の解析等を駆使して、包括的な探求を行った。その結果、血液腫瘍の分野において極めて予後が不良である赤白血病に関して全ゲノムシーケンスによるゲノムの構造異常解析を行うことで新規の治療ターゲット分子を発見し標的治療法を開発した。また、がん発症以前の先天性のリスクを明らかにするため胚細胞性変異の解析を進めることにより、成人の急性骨髄性白血病における最大の先天性リスクを個別のリスクアレルごと明らかにし、そのリスクアレルを持つ健常者における将来の発症率を定量した。

Current Status of Research Progress

Reason

がんの初期発生の解明に関しては大腸癌において、前がん状態では認められるものの大腸癌ではほとんど認めない、がんになりにくい遺伝子異常を同定した。この知見は、初期発生の解析によるものであるが、がんの治療標的候補にもつながりうる、想定した以上の非常に重要なものである。また、クローン造血の解析では、当初想定していなかった大規模な健常者サンプルと臨床情報が得られ、さらに大規模のSNPアレイデータの利用により、最新の超高感度コピー数解析を行うことが可能となった。その結果、クローン造血における遺伝子変異とコピー数解析の両者がともに、造血器腫瘍および非腫瘍性疾患のリスク増大に重要であることが判明した。
さらに、当初は細胞レベルでの遺伝子変異と遺伝子発現の解析を単一細胞由来コロニーで解析する予定であったが、本研究において新規に開発した単一細胞での変異と発現同時解析技術の最適化が進み、培養を経ることなく、単一細胞レベルで遺伝子変異と遺伝子発現の同時解析が可能となり、クローン性造血の解析を進めている。MDSの解析では、我々が長年取り組んできた大規模コホートのゲノム解析によって、DDX41の胚細胞変異による先天性の発症リスクの評価をおこなった。その結果、我が国で認められるリスクアレルをもつ健常者が将来MDSを発症するリスクを定量することができた。
また、変異陽性症例において治療の有効性に関する重要な知見を得ることができた。これらの当初の期待以上の成果が得られている項目に加えて、きわめて予後不良な急性白血病である赤白血病などの疾患に関しても順調に研究成果が得られている。正常組織に関しては引き続き乳腺・尿細管・子宮上皮などにおいて薄切病理標本を用いて組織の構造を確認した上で微少クローン組織を採取して詳細なゲノム解析を進めている。

Strategy for Future Research Activity

昨年度までの成果を踏まえ、がんにおいて高頻度に認められるドライバー変異が獲得される時期およびそれらの組み合わせや時間当たりの獲得頻度などの詳細を、臨床経過と関連させて解析する。その結果に基づいて、ドライバー変異の段階的な蓄積が初期のがんの発生過程にどうかかわるか、また、がんが高度に多様な細胞から構成され、他の臓器に浸潤や転移し、また臨床経過中に病期が進展する過程の分子メカニズムについて明らかにしていく。これまでに、がんのドライバー変異はすでに発症前から獲得されていること、がんにおけるそれらの検出頻度は正常組織とパターンが異なり、遺伝子によっては、正常組織に認められるドライバー変異ががん組織で認められるよりも高頻度であることを見出した。この現象が生じるメカニズムおよび、組織のリモデリングとの関連を検討し、食道がん、大腸がん、造血器腫瘍、上部尿路上皮がんにおいて、がんおよび正常組織における遺伝子異常の検出、発症初期のメカニズムの解明、新たな病態の解明など、大きな成果が得られた。その際に、がんの起源である正常組織における遺伝子異常を検出すること、遺伝子異常やその組み合わせと細胞のフェノタイプとの関係性を検討することを可能とした、ごく少量のサンプルからさまざまなシーケンス解析を行う独自の技術についてレーザーマイクロダイゼクションを含めさらなる最適化を進めている。この技術をさらに多くのがんに適用し、これまで未知のゲノムの異常を解明することを試みるともに、大規模コホートでの遺伝子異常と表現型の解析や、変異クローンの進化や拡大の履歴推定などの解析を行う。現在、研究が予定されているがんとして、乳がん、胃がん、大腸がん、腎がん、子宮がん、脳腫瘍、造血器腫瘍が含まれる。これまで本研究は予定以上の成果をあげており計画の変更あるいは問題点を認めていない。
