Prediction of therapeutic effect of cognitive behavioral therapy for anxiety and obsessive-compulsive disorder

Outline of Annual Research Achievements

認知行動療法は、患者の認知や行動をより適応的なものへと変容させていくことを援助する治療法であり、うつ病、社交不安症等の不安症、および強迫症などに対して薬物療法に劣らない治療効果を発揮することがわかっている。しかしながら、本邦の医療現場において、認知行動療法を希望する患者全員に提供できる医療機関や治療者の数は依然として限られている。そこで、認知行動療法が奏効する可能性が高いと判断される患者に優先的に認知行動療法を提供していくために、あらかじめ治療反応性を予測する手法を確立することを目的とした。本年度は、認知行動療法を受ける予定の不安症9名、強迫症7名、健常対照者30名をリクルートし、症状評価、MRI検査、認知機能検査を実施し、認知行動療法を完遂した患者には、治療後の症状評価を行った。また、認知行動療法の治療効果予測の候補因子の探索のため、社交不安症を対象とした認知行動療法の治療効果と関連する安静時脳機能結合を探索した。MVPAによる解析により抽出された左視床と右前頭極・下前頭回間を結ぶ安静時脳機能結合が社交不安症の重症度変化と有意に相関していたことから、個人認知療法の有効性を示す指標となる可能性が示された。本結果は、2023年12月に国際学術誌Frontiers in Psychiatryに受理・掲載された。また、強迫症に関しては、30名の患者を対象にしたMVPA解析により、CBT施行前のrsfMRIにおいて後頭皮質を中心とする機能的結合性と、Y-BOCSの変化量に相関が認められた。このクラスターを元にSeed to Voxel解析により得られた7つの脳機能結合をBorutaにより選択された特徴量により認知行動療法の治療予測モデルを評価した結果、比較的良好な性能を示した（AUC=0.77）。本結果は、日本認知療法・認知行動療法学会で報告された。

研究開始初期の新型コロナウイルス感染症の蔓延にともない、認知行動療法を受ける不安症と強迫症患者のリクルートが遅れたため。

Strategy for Future Research Activity

患者と健常対照者のリクルートと症状評価、MRI検査、認知機能検査を引き続き行うとともに、前年度までに得られた各種MRI画像と心理指標と治療効果との関連性を調べるためのデータ整備を行うとともに、集積したデータを用いて解析を進める。

Causes of Carryover

研究協力謝金と心理検査者謝金の低減が可能であったため、次年度使用額が生じた。
特任教員・研究員の人件費、研究成果を学会で発表するための旅費と学会登録費、英文校閲費、論文掲載料として使用する予定である。
