Outline of Annual Research Achievements

大腸の腫瘍性ポリープは前がん病変であるため、摘除が推奨されている。しかし、内視鏡での腫瘍/非腫瘍の鑑別精度は80%台に留まっており、不必要なポリープ切除が横行している。申請者は人工知能(AI)を利用することで、この問題を解決できると考え、複数の科研費研究を経て、95%超の感度・特異度で腫瘍を鑑別しうる自動診断システムを開発した（Mori, et al.Annals of Internal Medicine 2018）。
本研究では、国際共同研究という、より厳しい環境下でも、今までに得られた成果が再現されるかどうかを確かめることを一義とする。具体的には、オスロ大学（ノルウェー）・キングスカレッジ（イギリス）・昭和大学（日本）での3か国による国際共同研究を立案、実施した。デザインは、前向きの優越性検証試験である。主要評価項目を微小腺腫診断における感度とし、大腸内視鏡中に人工知能を用いた場合と用いない場合での診断能の差を評価した。倫理委員会承認を取得、臨床試験登録をした後（UMIN000035213）、2019年度始から患者リクルートを開始した。2020年3月現在で711人の患者集積が終了している。なお、研究に伴う有害事象等は発生していない。

Reason

1年間で集積された患者数は、既に全集積予定患者数の2/3にあたるため、研究は想定以上のスピードで進捗していると考えられる。

Strategy for Future Research Activity

2020年度も引き続き患者リクルートを継続し、人工知能の実臨床における性能を国際共同研究というエビデンスレベルの高い形で公表できるよう、研究をすすめる。

Causes of Carryover

当初想定していたよりも、論文英文校正代金が安価に済んだため、次年度使用額が生じた。当該助成金は翌年度における、国際共同ミーティング費用の一部に充当する予定である。
