Outline of Final Research Achievements

We conducted a single-center retrospective observational study to investigate the association between intraoperative respiratory management and postoperative delirium in 2,695 adult surgical ICU patients. We found intraoperative hypocapnia was independently associated with postoperative delirium. Our findings support the need for future clinical trial to investigate whether intraoperative targeted mild hypercapnia would reduce the incidence of postoperative delirium in patients undergoing general anesthesia.

Academic Significance and Societal Importance of the Research Achievements

術後せん妄の発生は、患者さんの長期的な経過に悪影響を及ぼす可能性が報告されており、その予防・治療に関する研究が世界中で活発に行われている。本研究では、術中の低CO2血症の発生と術後せん妄の発生に関連があることが明らかとなり、次のステップとして術中の低CO2血症を予防するプロトコールの導入の効果を検討する介入研究を行うことの妥当性を示すことができた。次に計画している研究で、術中CO2管理によって術後せん妄の発生を予防することができれば、患者さんの長期的な経過の改善や医療費の削減につながる可能性がある。
