Research Abstract

1.医療安全全国共同行動がめざす8つの安全目標に取り組む病院への支援活動(支援ツールの提供、研修の実施等)を実施するとともに、推奨対策実施度のモニタリングとアンケート調査によって、対策実施上の困難点の把握に努めた。
2.入院死亡数の増減累積和の算出:A.全病床・全年齢と、B.一般病床・75歳未満を対象として、粗死亡数(死亡退院数の実数)の増減の累積和を算出した。
Aは2007年4月から各2か月の退院死亡数を参照値とし,Bは2008年の2カ月平均値を参照値として、2008年、2009年の2カ月ごとの退院死亡数との差を算出し、これを合計したものを、全病床・全年齢の粗死亡数の増減累積和(A)および(B)とした。在院期間の短縮が進んだことで対象期間中の入院患者数が全施設合計で大幅に増加したため、入院患者数が(A)2007年度の参照月、(B)2008年度の2カ月平均値、とそれぞれ同じだったと仮定した場合の死亡数(調整後死亡数)の増減累積和を算出した。現在、病院の機能、規模および対策実施度との関係の分析を進めている。
3.日本版標準化病院死亡比(HSMR)の算出:協力が得られた70の有志病院について、患者側のリスク因子(疾患構成他)を調整した標準化病院死亡比(Hospital Standardized Mortality Ratio ; HSMR)の算出を試み、英米例と比較しながらDPCデータによるHSMR算出の方法および妥当性の検証を行った。その結果、(1)24時間以内の死亡を除外することで英米と比較可能なHSMRの算出が可能、(2)日本の病院は規模と機能で多様性が広く、かつ急性期医療と慢性疾患が混在するため、類似病院群別に死亡比を算出することが実用性を高めると考えられた。
