Outline of Final Research Achievements

In the first year, we developed a prototype "automatic lymphatic fluid guidance system in the thoracic duct" and succeeded in automatically guiding low-flow lymphatic fluid out of the body. In the next year, the system was modified from a structure in which a certain amount of lymph fluid is stored in a Pouch outside the body and then returned to the body, to one in which lymph fluid is continuously guided out of the thoracic duct and returned into the vein. In the final year of the project, we conducted two animal experiments using pigs and introduced the above system in vivo. We established a method for lymphangiography in pigs, a method for thoracocentesis, and a method for retrograde cannulation of the thoracic duct. We also succeeded in creating a model of thoracic duct obstruction to create a model of increased intra-arterial pressure in the lymphatic vessels and successfully introduced the device. The plan was completed in this research period.

Academic Significance and Societal Importance of the Research Achievements

心不全におけるリンパうっ滞に対する治療は、Medicationのみだが、本研究によりリンパうっ滞によって生じたリンパシステム内のリンパ液の能動的導出し、大静脈への排出が可能となった。これにより、先天性心疾患にしばしば合併するリンパうっ滞症候群であるFontan-related lymphorreha(Chlothorax, Chylopericardium, Protein-loosing enteropathy, Plastic bronchitis)の治療につながることが期待される。また、急性、慢性心不全に時に合併する高度なリンパ浮腫の治療につながることが期待出来る。
