Outline of Annual Research Achievements

半導体デジタルPET/CT(dPET)は,従来型(cPET)と比較して感度および分解能が優れている. dPETとcPETの病変検出能やstandard uptake value (SUV)などの定量値を,心血管系の炎症性疾患150例で比較した.新旧の2機種を用いて,撮影順序は無作為に連続撮影した.左室心筋へのFDG集積は,dPETにおいてSUVmax,バックグランド比(TBR), 視覚評価のいずれも高かった.生理的集積は,低炭水化物食に加え,18時間以上絶食を前処置とすることで完全に抑制された.両心房のSUVmaxはdPETで高値で,視覚的検出率も高かった(左房:16% vs.7%, p=0.0067).CT冠動脈造影とdPETの融合画像での検討では,目標とする冠動脈および周囲脂肪へのFDG集積の検出は空間分解能が及ばず困難だった.冠動脈周囲脂肪への集積は,IgG4関連疾患や心房細動などで検出されたが,高度石灰化を有する冠動脈病変での検出率は低かった.一方,中等度血管(外径5-8mm)の炎症である結節性動脈周囲炎では, 検出能の向上が確認された.次に,大血管炎(外径2-3cm以上)の活動性を,治療開始前の患者について評価した.高安動脈炎においてはNIHスコア2点以上,巨細胞性動脈炎ではCRP 1.0mg/dL以上を活動群(N=19)と定義して,SUVmaxとTBRを非活動群(N=11)および対照群(N=30)と比較した.活動群のSUVmaxの中央値は,5 (3.5-6.1),TBRは3.2 (2.3-4.4)で,他の二群と比べて高値だった.活動群のSUVmaxを用いた診断能についてのROC解析を施行したところ,AUCは0.94, SUVmax 3.4をカットオフ値とすると感度84.2%, 特異度90.1%と, 大血管炎の活動性の指標としては極めて有用性の高い指標となった.
