Outline of Final Research Achievements

The histological change was evaluated in rat lungs that underwent mechanical ventilation through a tracheostomy. There was a distribution difference in pro-inflammatory cytokine expression among lung areas. Cytokine expression was also different in lungs with different tidal volume ventilation. We confirmed a distribution difference in lung injury among lung areas, which gives ground for research on lung injury according to transpulmonary pressure.

人工呼吸の設定を決める際に、どのような換気量が適しているか、呼気終末陽圧(positive end-expiratory pressure, PEEP)をいくつにしたら良いか、プラトー圧以外に何を指標にすべきかなど、未だコンセンサスが得られていない。近年driving pressureや経肺圧を指標に呼吸器設定することが提案されている。経肺圧も肺の部位によって異なるため、その部位ごとの値の違いを知ることが、今後の呼吸器戦略における重要な布石になると考えられる。
