Research Abstract

未知の病型の遺伝性脊髄小脳変性症の新規原因遺伝子同定を目指して下記の研究を行った。当施設で集積した常染色体優性遺伝形式を示す脊髄小脳変性症家系のうち、既知の病型(SCAI・SCA2・MJD/SCA3・SCA6・SCA7・SCA8・DRPLA)を除外した39家系中、神経学的評価および脳画像解析上、均一の臨床表現型を示し、地理的にも隣接した地域出身の10家系を対象として解析した。これらの家系においては同一の遺伝学的背景をもつ可能性(創始者効果)も想定された。多型性DNAマーカーを用いて全ゲノム領域を対象とした連鎖解析を施行した結果、染色体上の候補領域は数か所に絞られた。今後は同領域内の候補遺伝子を検討し、優先的に遺伝子変異検索を行う予定である。
脊髄小脳変性症の臨床病態研究については、spinocerebellar ataxia type 31 (SCA31)および他の病型の小脳失調症における聴覚障害について詳細な検討を行った。既報の論文結果とは異なり、聴覚障害はSCA31に特徴的とは言えず、病型の鑑別診断に役立つような小脳外臨床症候とは言えないことを明らかにした。また、本州最北の青森県地域の遺伝性脊髄小脳変性症の臨床遺伝学的特徴を明らかにした。以上の研究成果は、脊髄小脳変性症の根本的治療法の開発へ向けて重要な知見を与えるものと考えられた。
