難治性EBウイルス関連疾患の病因と治療に関する研究

Research Abstract

難治性EBウイルス関連疾患であるEBウイルス関連血球貪食性リンパ組織球症(EBV-HLH)と慢性活動性EBウイルス感染症(CAEBV)の治療法確立をめざして、(1)造血細胞移植の成績と(2)EBV持続感染細胞の特性について解析した。この2つの疾患は、日本に多い予後不良なEBV感染T/NK細胞リンパ増殖性疾患である。(1)過去20年に行われたEBV-HLH 14名と家族性HLH(FHL)43名に対する計61回の造血幹細胞移植(HSCT)の成績を解析した。診断はEBV DNA量、PRF1およびUNC13D変異を基本とした。半数に非血縁者間臍帯血移植(UCBT)が、1/4に骨髄非破壊的前処置が行われた。10年生存率はEBV-HLH(86%)がFHL(65%)より良好であった。FHLはEBV-HLHより100日以内の早期死亡(7名)と神経学的後遺症が多かった(p<0.02)。UCBT後の生存率は、いづれのHLHも65%をこえ、2回UCBTをうけた4名中3名も生着生存した。EBV-HLHでは移植のみが根治療法となるFHLとの正確な鑑別が必要であること、UCBTはいずれのHLHにも有用性であることを明らかにして報告した。今後、UCBTの至適前処置の検討が必要である。(2)CAEBV患者(T細胞型3例、NK細胞型3例)の高純度分離細胞を用いてその感染細胞源を検討した。主たる標的細胞がいずれであってもそれ以外の細胞亜群にEBVまたはT細胞レセプターレベルでのクローン増殖を確認した。さらにEBV-TRの同一性からT/NK前駆細胞での感染が示唆された。しかし、骨髄の造血前駆細胞の感染はまだ確認されていない。この解析を継続しながら、感染細胞亜群の移入実験を2例で開始した。
