| Research Abstract |
難治性EBウイルス関連疾患のうち、慢性活動性EBウイルス感染症(CAEBV)についてはT細胞型とNK細胞型の患者における感染細胞源の解析を終了した。高純度分離細胞とSouthern blot解析を用いて、同一患者において主たる標的細胞ではない細胞亜群にもEBVが感染しクローン増殖していること、さらにEBV-terminal repeatsの同一性からT細胞とNK細胞の前駆細胞に感染していることを示した。一方、骨髄のCD34陽性造血前駆細胞は感染から免れていた。この結果はEBVのT/NK細胞への感染機構に関する知見としてJ Clin Virolに受理された。この感染細胞亜群の免疫不全マウスへの移入実験を進めている。EBウイルス関連血球貪食性リンパ組織球症(EBV-HLH)と家族性HLHについては、造血細胞移植の成績を米国小児血液学会誌に報告し、2010年アジア太平洋骨髄移植学会などで招請講演を行った。海外に比較して、日本ではEBV-HLHとFHLHの何れにも臍帯血移植が多く行われていたが、10年生存率はEBV-HLH 86%、FHLH 65%と比較的良好であった。しかし、移植後早期の治療関連死亡と中枢神経後遺症が問題であり、骨髄非破壊的前処置の有用性を明らかにすることはできなかった。最近、EBV-HLHとFHLHの確定診断が早くなり、適切な移植時期と前処置の強度を検討することが課題となっている。2006年以降の移植例を調査し、この2点を中心に治療法の検討を開始した。また、EBV-HLHの病因に関しては感染細胞のマウスモデルでの解析を計画している。
