プロテインアレイシステムを用いた小児劇症肝不全の病態解明

Research Abstract

2005年11月から2009年8月までに国立成育医療センターで生体部分肝移植を行った92例のうち、劇症肝不全症例17例について検討した。男児13例、女児4例、移植時月齢は1から87カ月で、中央値は14カ月であった。16例は原因不明で、1例はサイトメガロウイルス封入体が摘出肝にて認められた。17例中1例は移植後74日目に敗血症により死亡したが、16例は生存している)。摘出された肝組織8例について、サイトカインアレイ(RayBio)を用い、42種類のサイトカインの発現を測定し、正常肝と比較した。劇症肝不全では、5種類のサイトカインの発現が高く、これらのサイトカインは、正常肝では発現がほとんど認められなかった。最も発現の高かったサイトカインAについて、凍結肝組織を用いた蛍光抗体法にて発現を検討したところ、類洞内皮細胞に発現しており、小葉内胆管、中心静脈、門脈壁には発現が認められなかった。
Desorption/Ionization(SELDI)プロテインチップシステムにより、小児劇症肝不全において有意に発現しているタンパクを同定するため、測定条件について基礎的な検討をした。
