食道癌発癌における喫煙・飲酒による酸化的DNA損傷および癌抑制遺伝子異常の関与

| Research Abstract |
1.食道癌発癌における酸化的DNA損傷およびその修復系の関与
8-oxoGは酸化的DNA損傷の指標でありOGG1で塩基除去修復される。一方、MutYHは細胞死を誘導する。食道癌切除例でOGG1と8-oxoG、MutYHの免疫染色を行った結果、1)喫煙指数が多いほど食道上皮で8-oxoGが蓄積していた。2)食道上皮に対し癌部ではOGG1の発現が低下していた。3)正常食道では8-oxoGとOGG1の発現は正の相関を示したが、癌部では相関を認めなかった。4)MutYHは正常上皮ではほとんどの症例で検出できなかったが、癌部では過半数で発現をみとめた。5)MutYH強発現例で腫瘍径が大きい傾向にあった。以上より、タバコの暴露によって食道粘膜に酸化ストレスが惹起されるが、癌部ではOGG1の発現が低下によりDNA修復の障害が考えられる。さらに、癌組織では異常MutYHによる過剰発現が認められ、細胞死が障害されている可能性が示唆された。
2.食道癌におけるp53異常に注目した発癌分子機構の解析
p53遺伝子の食道癌の発癌における意義を明らかにする目的で、食道扁平上皮癌における遺伝子変異、LOHを解析した。その結果、61%にp53遺伝子変異を認めた。その変異スペクトラムは他の癌腫と異なり、喫煙、酸化ストレスが関与するトランスバージョン変異が優位であった。一方、p53遺伝子座のLOHを68%に認めた。また、p53遺伝子変異とLOHの間に有意な相関を認めた。食道癌におけるp53機能異常は、遺伝子変異、LOHともに高頻度であり、点突然変異及び染色体異常の両方がその発生に関与している。これらのp53の異常は食道発癌において重要であると考えられる。
