肝虚血再潅流障害に対する分子細胞標的治療法の開発

Research Abstract

Rhoキナーゼ阻害剤を肝星細胞に効率よく運搬する新たなDrug Delivery Systemを作製し、肝虚血再潅流障害の分子・細胞標的治療法を開発することを目的に研究を行い以下の実験結果が得られた。
1.肝星細胞に親和性の高いビタミンAをリガンドとしてRhoキナーゼ阻害剤を内包化したリポソーム体を作製し、その内包化率を液体クロマトグラフィーにより測定し、至適配合比率を決定した。
2.同リポソーム体の肝星細胞への取り込みを確認するため、Rhoキナーゼ阻害剤の代わりにGFPを内包化させ、ラットに同リポソーム体を静注した。肝への取り込みは心・肺・腎などの他臓器に比較し有意に高かった。また、肝星細胞への特異性を確認するため、デスミンによる二重染色で類洞壁に高い特異性を持ってGFPが取り込まれていることを確認した。
3.分離したラット肝星細胞への同リポソーム体の作用を確認するため、collagen gel-contraction assayを用いて検討した。Rho阻害剤単身とリポソーム体で比較すると、同リポソーム体は約100分の1の濃度で非リポソーム体と同程に肝星細胞の収縮を抑制した。
4.ラット虚血再潅流障害モデル(全肝45分阻血)を作製し、同リポソーム体とRho阻害剤単身の効果を比較すると・上記collagengel-cohtractionassayの結果と同様に、同リポソーム体はRho阻害剤単身の約100分の1の投与量で術後3時間のGOT値を同程度に抑制した。
5.ラット虚血再潅流障害モデル(全肝70分阻血)での実験では、未治療群はラットの1週間生存率では0%であったが、Rho阻害剤単身では10mg/kgで100%生存を得られた。一方、同リポソーム体では0.1mg/kgで100%生存が得られた。つぎに同リポソーム体によるRho阻害剤の副作用(低血圧)抑制効果を確認した。Rho阻害剤単身の投与では1mg/kgで体血圧は平均90mmHgから約40mmHgへ低下するも、同リポソーム体ではラット虚血再潅流障害モデルにおける抑制効果が得られた0.1mg/kgでは体血圧は平均90mmHgから約70mmHgへの低下にとどまった。
以上の結果から、Rho阻害剤を内包化したビタミンAリポソーム体は、Rho阻害剤単身の投与に比較し、約100分の1の投与量でラット虚血再潅流障害を改善させることが判明し、さらにRho阻害剤の投与量が少ないことで低血圧などの副作用も軽減することができた。
