腎部分切除術後の腎機能回復に及ぼす自己脂肪組織由来幹細胞の有用性に関する研究

Research Abstract

この研究は腎細胞癌症例における血管遮断を行い腎部分切除時に生じる虚血性腎障害を臨床再生治療、脂肪由来幹細胞用いていかに防ぐことが可能かポイントとなる。その治療の新しい臨床手技として自己吸引脂肪から脂肪由来幹細胞を細胞分離装置で抽出することにある。その手技は下部尿路の臨床再生治療として前立腺癌術後難治性腹圧性尿失禁治療を我々のグループは行い短期間の有用性を報告している。よって、その頻度の詳細を検討するために前立腺癌術後の合併症としてめ尿失禁の患者満足度の全国調査を行った。
本教室は腎細胞癌の腎臓摘出術は全国第三位の頻度で行っており10-20%は腎動脈遮断腫瘍部分切除術を積極的に行っている。血流と腎臓再生の臨床検討を行うために腎臓虚血の慢性状態にあたる血液透析中に発生した腎細胞癌の調査を行った。
また、腎尿細管周囲毛細血管血流評価を時空間画像処理で行い、コントロール群約0.6-0.8mm/secであることを明らかにした。
臨床の症例における腎細胞癌に対する腎血管一次遮断(15-40分虚血)腎部分切除を想定とした片腎摘虚血ラットモデルを用いて皮膜下に脂肪由来幹細胞注入を行い、腎機能保護作用を確認しその有用性としては注入した脂肪組織由来幹細胞から分泌されるサイトカイン(HGF,VEGF)であることを明らかにした。今後、臨床脂肪幹細胞研究を下部尿路疾患として腹圧性尿失禁症例に行っており、この基礎研究を臨床へ応用するための検討が必要である。
