Research Abstract

自閉症の社会認知障害の臨床的特徴の1例として、「人の目を見ない」ということがあげられる。人の顔を見せた時の注視点を簡便に測定できる装置があれば、自閉症の早期診断ツールとして利用できる可能性がある。そこで、静岡大学、株式会社タイカとともに、視線計測装置を開発した。この視線計測装置を用いて、年齢、性別をマッチングさせた自閉症児童群と定型発達群(年齢7-12歳、両群ともn=18)で、人の顔を見せた時の注視点分布を解析すると、定型発達児童群に比べて、自閉症児童群の注視点が目の領域に集まりにくいことが明らかとなった(p<.0001)。また、目の領域への注視時間を用いて判別式にかけたところ、感度94%、特異度83%であった。
さらに、HBC (Hamamatsu Birth Cohort)研究に参加し、2010年6月30日までに生まれた844名の児を対象とした中途解析結果を、以下に示す。844名(男児435、女児409)の平均出生体重は2949g、うち103名(12%)がLBW (low birthweight)であった。対象児を24ヶ月まで追跡し、社会認知機能の諸指標をLBW児およびNBW (nomal birthweight)児ごとに集計して比較したところ、(1)社会認知の発達指標到達度では「『ダメ』に対する適切な反応がみられる(14ヶ月)」(LBW 43% vs NBW 58%)、「『○○をママにあげて』に従える(14ヶ月)」(11% vs 21%)、「呼名反応がある(18ヶ月)」(86% vs 97%)、「共同注視に反応する(24ヶ月)」(83% vs 97%)における到達度に有意な相違が認められた。また、(2)社会行動の評価得点では、「社会的ジェスチャー数(10ヶ月)」(LBW 3 vs NBW 4)(中央値median比較)「および「同(14ヶ月)」(7 vs 8)、「使用可能な道具数(10ヶ月)」(1 vs 2)および「同(14ヶ月)」(7 vs 9)、「総ジェスチャー数(10ヶ月)」(5 vs 7)および「同(14ヶ月)」(23 vs 29)で有意な差がみられた。総ジェスチャー数を指標とすると、出生体重と正の相関が認められた(10ヶ月r=0.16、14ヶ月r=0.15)。以上のように、中途段階での解析において、LBW児に社会認知機能における遅延が認められている。

引き続きHBC研究を維持すると共に、HBC研究から得られたデータを集計し、最終的な解析にあたる。結果のまとめにとりかかる。
