抗リン脂質抗体症候群の発症メカニズムの分子病態論的解析

Research Abstract

本研究では、抗β2-GPI抗体と結合する分子の同定を行い、細胞表面の受容体、共役分子などの機能について幅広い解析を行って、抗リン脂質抗体の病原性を分子生物学的に解明することが目的である。これまでの解析によってβ2GPIのヒト血清中における主要な結合タンパクは酸化LDLと考えられ、酸化LDLが抗リン脂質抗体症候群における血栓形成病態に関与する可能性が示唆された。また、酸化LDL関連分子のリポプロテイン関連フォスフォリパーゼA2(LpPLA2)が、抗リン脂質抗体症候群患者に高発現することを見出した。本年度は酸化LDLが抗リン脂質抗体症候群患者に高発現する要因を調べるため、LpPLA2関連遺伝子を中心に、遺伝子多型解析をおこなった。LpPLA2関連GWASより同定された5個のSingle Nucleotide Polymorphism(SNP)について解析した。SNP解析にはAPS 91名(PAPS 43名、続発性48名)、APSを合併していないSLE 138名、健常人378名の血液から採取したDNAを用いた。5個のSNPのうち、APOE/APOC1 (rs4420638 A/G、第19染色体) 、LpPLA2(rs1805017 A/T、第6染色体)の2個のSNPがAPSと関連していた。rs4420638 AG+GG遺伝子型頻度はAPS患者に有意に多く(OR[95%C.I] = 1.69 [1.02 - 2.79], p=0.0038)、rs1805017 CT+TT遺伝子型頻度はAPS患者に有意に少なかった(OR[95%C.I] = 0.36 [0.18 - 0.71],p=0.0028)。これらの結果からAPS患者に酸化LDLが高発現する理由の一つにSNPの関与が示唆された。
