| Research Abstract |
まず、IgA腎症患者の扁桃特異的細菌叢の同定を行った。大阪大学腎臓内科にて腎生検を行い診断が確定し扁桃摘出ステロイドパルスを行った患者は、およそ80名おり、このうち既に治療後2年経過し、扁桃摘出およびステロイドパルス療法により完全寛解に至った患者は25名である。また、同治療では残念ながら寛解に至らなかった患者がおよそ20名いる。また、コントロールとして慢性扁桃炎にて扁桃摘出を行った患者の扁桃の組織は25名分現在保存している。これらの扁桃に特異的な細菌叢をDDGE法を用いて検出を行った。さらにIgA腎症群で有意に検出率の高い細菌に相当するバンドを切り出してシーケンスを行うことで菌種の同定を行いTreponema sp., Haemophilus segnis, Campylobacter rectusであることが同定された。さらに扁桃摘出の術前評価に向けて扁桃のぬぐい液での検出法の確立を行っており、具体的には扁桃のぬぐい液を用いて、決定された細菌叢にと区的なプライマーを用いてＰＣＲを行うことで同定する方法の確立を行い、予備実験で少数で検出が可能であることを確認した。また、IgA腎症患者の扁桃特異的細菌叢と扁桃摘出＋ステロイドパルス療法の治療効果との関連性についての検討を行い、Treponema sp., Campylobacter rectusが、扁桃摘出＋ステロイドパルス療法の治療効果と強い相関があり、治療予測能があることを確認した。
この結果を踏まえ、治療効果と相関のあるTreponema sp., Campylobacter rectusの２種について抗体を作成した。菌の扁桃の分布を確認したが腎臓においては検出されなかった。また、嫌気性菌であり培養が一般に困難な細菌ではあるが、これらの菌をマウスに感染させることでIgAの腎臓への沈着を再現することが可能か検討を始めている。
