Outline of Final Research Achievements

The purpose of this study was to develop a method to detect the risk of developing aspiration pneumonia, which signs are vague, using the serum antibody titer of oral bacteria as an indicator.
Serum antibody titers of oral bacteria in patients with aspiration pneumonia were compared with those in healthy elderly and young healthy individuals. Among the analyzes conducted so far, the serum antibody titer of Streptococcus mitis in healthy young people tended to be lower compared to those of healthy elderly and aspiration pneumonia patient groups. If an increase in S. mitis serum antibody titer is associated with a decline in swallowing function, it may be an indicator of "aging". The serum antibody titer of C. albicans in patients with aspiration pneumonia was significantly higher than those in healthy young and healthy elderly groups. It was a fungus that characterizes aspiration pneumonia.

Academic Significance and Societal Importance of the Research Achievements

誤嚥性肺炎の発症とともに血中の抗体価が上昇する口腔細菌が存在した場合、指尖から採取した微量の血液から誤嚥性肺炎の兆候を感知できる可能性がある。地方で進む過疎化や医師偏在により、近くに病院がない高齢者が増加している。本システムが実用化されれば、高齢者の体調が普段と違う時に、「病院へ連れて行ったほうが良いか、自宅で薬を飲ませて様子を見て良いか」を判断するツールになりうる。検査は訪問看護師が実施することを想定している。本手法の確立は、在宅医療にとどまらず、遠隔医療、僻地医療、災害医療でも有効なツールとなるだろう。
