Research Abstract

目的:筋小胞体(SR)に存在する心筋型Ca2+放出チャネル(リアノジン受容体:RyR2)内の構造連関の安定化により、遅延型後脱分極(DaD)から致死的不整脈につながる拡張期の局所的Ca2+濃度上昇を抑制し新たな心不全・心肥大・致死的不整脈の包括的治療法を確立すること。
結果:1) R2474S+KIマウスでは、CaMのRyR2に対する結合親和性を著しく高めたGSH-CaMはisoで誘発したCa spark頻度を著明に抑制した。2)R2474S+KIマウスの心筋細胞ではGSH-CaMの導入でafter transientやDADの出現を押さえることができた。3) TacモデルマウスにおいてもCaMの親和性の低下が示唆された。
以上、CPVTのCaハンドリング異常のメカニズムとして、RyR2内ドメイン連関障害→CaM解離→Ca2+ 漏出が示唆され、さらに心不全においても同様のメカニズムで不整脈を生じることが強く示唆された。高親和性のGSH-CaMでCa2+ 漏出を防ぐことにより新たな心不全・不整脈治療が可能となることが示唆された。

Strategy for Future Research Activity

Tacモデルでの知見をさらに確認する。新たなCPVT型KIマウス(C末側のmutation)が今年度前半で作成完了予定であり、さらなる研究上の推進が期待できる。
