| Research Abstract |
1)弁尖デザインの改良(早稲田大学)
循環シミュレータを用いて弁尖の形の改良を継続した。動物実験ならびに実際の臨床にて得られた心エコーデータを基に、前尖の形態を改良し、僧帽弁を通過する流入血流のダイナミックスを改良中である。
2)動物実験(大阪大学)
大動物(ブタ)を用いて動物実験を10例行った。術後急性期の心エコーデータは6例にて収集可能であった。ステントレス僧帽弁の急性期において逆流は認めず、流入血行動態も概ね良好であった。
3)ウェットラボによる手術手技の確認
平成23年度7月、12月の2回ブタの心臓を使ったウェットラボを開催し、手術手技の確認、手技のトレーニング、手技上の問題点を検討した。またステントレス僧帽弁の手術適応についても討論を行った。
4)臨床応用(榊原記念病院、京都府立医科大学)
平成23年度に新しいステントレス僧帽弁を用いて5例の患者に対して手術を行った(榊原記念病院4例、京都府立医科大学1例)。手術適応は本研究の代表研究者、分担研究者、研究協力者を中心とした検討会にて慎重に検討し、既存の弁置換術では十分な効果が得られないと考えられた症例に手術がなされた。各施設での倫理委員会の承認は取得済である。いずれの患者も経過順調で、僧帽弁の逆流は無く、また流入血行動態も良好であった。現在最長フォローが1年未満であるが、更なる追跡を行い遠隔成績のデータを蓄積していく。

Reason
当初の研究計画では2～3年目(平成24～25年度以降)に臨床応用を行う予定であったが、各施設での倫理委員会の承認取得、手術適応の決定に関する討論が順調に進み、またステントレス僧帽弁の手術適応に合致した患者も出現したため、臨床応用を早めることができた。

| Strategy for Future Research Activity |
1)循環シミュレータによる弁尖デザインの改良:臨床データならびに実験データに基づき弁尖の形態の更なる改良を加え、循環シミュレータにより評価を行ったうえで臨床に応用していく(早稲田大学)。
2)動物実験の遂行:さらに動物実験の数を重ね、蓄積したデータを論文、学会発表する(大阪大学)。
3)臨床経験の蓄積:今年はさらに手術件数を重ね、またより長い追跡データを蓄積していく(榊原記念病院、京都府立医科大学、東京慈恵会医科大学、大阪大学、神戸中央市民病院)。
4)ウェットラボによる手技の確認:平成24年度は3回のウェットラボを行い、手技の確認を行っていく。
5)研究会開催による社会への成績公表:循環シミュレータ、動物実験の結果報告、臨床例の症例報告、今後の問題点と展開を検討するために7月28日にオープンの研究会を開催する(東京国際フォーラム)。
