Research Abstract

研究初年度である今年度は、我々が独自に開発したOA指標を定量化する自動定章システムを用いることにより、本邦における地域代表制を有したROAD Study対象者3,040例の膝、腰椎、股関節レントゲン写真の計測を行い、膝、腰椎、股関節OA指標の確立を完了したので、代表的な指標を記載する。膝に関しては、男性[内側最小関節裂隙幅(mJSW;mm):3.22±0.96、内側骨棘面積(OPA;mm^2):1.12±4.07、大腿骨脛骨角(FTA;度);177.0±3.3]、女性[mJSW:2.65±0.95、OPA:3.76±9.87、FTA:176.6±4.11]であった。mJSW、OPAは、男女とも年齢と関連を認めた(p<0.05)。FTAは男性では年齢との関連を認めなかったが、女性では年齢と関連していた(p<0.05)。腰椎に関しては、男性[椎間高(mm);Ll/29.87±1.72、L2/3 11.1±2.1、L3/4 12.0±2.2、L4/5 12.1±2.7、L5/S 1O.3±4.1、圧迫骨折(%):9.1]、女性[椎間高(mm);L1/2 8.5±1.6、L2/3 9.6±1.8、L3/4 10.4±2.0、L4/5 10.8±2.4、L5/S 9.8±3.0、圧迫骨折(%):10.8]。椎間高は、男性のL5/Sを除いて、いずれも年齢と関連していた(p<0.05)。股関節では、男性[最小関節裂隙幅(mJSW;mm):2.68±0.49、Centraledgeangle(CE;度):32.8±7.3]、女性[mJSW:2.40±0.46、CE:30.5±8.1]であった。mJSW、CEとも年齢との関連はなかった。現在、英米コホート対象者のレントゲン写真計測を行っているが、研究初年度に本邦における膝、腰椎、股関節のOA指標の性別、年代別基準値を確立できたことは非常に意義が大きいと考えられる。

Reason

本年度は本邦における地域住民代表性を有したROAD Study対象者3,040例の膝、腰椎、股関節レントゲン写真の計測を完了し、年代別、性別における各OA指標基準値を確立するとともに、英米におけるレントゲン写真の計測も進行中であり、おおむね計画に沿って進行中である。特に研究初年度である今年度は、計測が煩雑にて従来成し得なかった膝、腰椎、股関節の関節裂隙幅や骨棘面積などのOA指標を、われわれが独自に開発した自動定量システムにて計測し、その基準値を解明することができた。これらは、今後の変形性関節症の予防対策を進める上で基盤となる重要な疫学指標であり意義は大きい。

本研究課題の計画変更はなく、次年度以降、当初の計画に従い、研究を進めていく。
