変形性関節症の重症度定量的評価の確立および危険因子の解明：日英米国際共同研究

Research Abstract

これまでに、我々が独自に開発したOA指標を定量化する自動定量システムを用いることにより、本邦における地域代表性を有したROAD Study対象者3,040例の膝、腰椎、股関節レントゲン写真および英国Chingford study対象者1518膝のレントゲン写真の自動計測を行い、本邦および英国のOA指標基準値の確立を完了するとともに、日英の国際比較により、骨棘形成が日本におけるOAの特徴であることが示唆された。本年度は、膝における骨棘および関節裂隙狭小化の危険因子およびQOLへの影響について検討した。その結果、女性においてビタミンＫ，Ｂは関節裂隙狭小化および骨棘形成の療法と関連していることが明らかになった。さらに、ビタミンＣは関節裂隙狭小化のみに、ビタミンＥは骨棘形成のみに関連していることが示唆された。さらに、職業因子として、ひざまずく仕事、しゃがみ込む仕事は、関節裂隙狭小化と骨棘形成の両方に関連していた。さらに、長距離を歩く仕事、重いものを持つ仕事は関節裂隙狭小化のみに関連していた。また、女性においては、関節裂隙狭小化と骨棘形成はそれぞれ独立して痛みやADL障害と関連していることが明らかとなった。さらに、男性では、関節裂隙狭小化は痛みに、骨棘形成はADL障害により影響を与えていることが示唆された。OAに関するこれまでの研究は、関節裂隙狭小化と骨棘形成を混在して評価するカテゴリ化る分類を用いて行われていたが、本研究では、初めて関節裂隙狭小化と骨棘形成を別個に定量評価し、その危険因子、QOLへの影響を解明した点で非常に意義が大きいと考えられる。
